Collins Catholic School Student and Parent Handbook is a vital guide for a successful year and we hope it answers many of the questions you may have about procedures, regulations or practices. The handbook is particularly important to middle school students because it clearly states the expectations and responsibilities of the parents/guardians and the students.

Athletics Physical Evaluation Form - All students planning on participating in DPL Sports through James L. Collins Catholic School are required have a Physical Evaluation prior to participation. The annual physicals can be completed with your family doctor. Please print off the approved Athletic Physical Evaluation Form, complete the medical history portion only and return to the school office. All personal information is kept confidential.
